Filing a Complaint with the US Health and Human Services Office for Civil Rights (OCR)

Patients or personal representatives may submit written complaints to the Office for Civil Rights regarding the privacy policies, procedures or practices of any HIPAA covered entity.

The OCR website has information on how to file a complaint including forms and contact telephone numbers for assistance.
