State disciplines health care providers

OLYMPIA ľ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In April 2008 the Nursing Assistant Program denied the application of nursing assistant applicant Stacy K. Erickson (NA00200058). She was convicted of petty larceny and theft. She cannot reapply for five years.

Chelan County

In April 2008 the Nursing Commission charged licensed practical nurse reactivation applicant Cathy M. Schell (LP00042947) with unprofessional conduct. She was convicted of theft and allegedly failed to disclose the conviction on her application.

Clark County

In April 2008 the Nursing Assistant Program charged certified nursing assistant applicant Stephanie M. Fecht (NC10101028) with unprofessional conduct. Her license to practice as a health care assistant has terms and conditions set against it.

In April 2008 the Nursing Assistant Program charged certified nursing assistant applicant Amanda Feiss (NAC.NC.60012327) with unprofessional conduct. In 2007 she was not allowed to reactivate her registered nursing assistant credential because she failed to cooperate with an investigation.

In April 2008 the Nursing Assistant Program charged certified nursing assistant Jennifer E. Martin (NC10057361) with unprofessional conduct. Her registered counselor credential was suspended in December 2007.

In April 2008 the Nursing Assistant Program terminated the probation order against nursing assistant Erika L. Waters (NC10096726).

Cowlitz County

In April 2008 the Nursing Assistant Program granted the application of nursing assistant applicant Bailey M. Giard (NA00190780) and placed her license on probation for 18 months. Giard was found guilty of residential burglary. She must comply with the terms and conditions set against her license.

Grays Harbor County

In April 2008 the Nursing Assistant Program charged nursing assistant applicant Jillian M. Bryant Matsen (NA00198233) with unprofessional conduct. She was convicted of hit and run of an attended vehicle. She allegedly failed to disclose the conviction on her application and failed to respond to department inquiries regarding the conviction.

Island County

In April 2008 the Board of Physical Therapy charged physical therapist Perry M. Gordon (PT00010404) with unprofessional conduct. He allegedly tested positive for cocaine during a pre-employment drug screening.

King County

In April 2008 the Unlicensed Practice Program issued a notice of intent to issue cease and desist order to Tegwith E. Patchell. She allegedly gave massage treatments to patients and does not hold a credential to practice as a massage therapist.

Kitsap County

In April 2008 the Registered Counselor Program granted the application of registered counselor applicant Jamie R. Wright (RC00058256) and placed her license on probation with terms and conditions for three years. She was convicted of possession of stolen property and possession of a controlled substance – methamphetamine. She is also known as Jamie Rayola Wilson.

Mason County

In April 2008 the Nursing Assistant Program charged certified nursing assistant Kaliana Renkel (NC10086282) with unprofessional conduct. She allegedly tested positive for drugs during a workplace urine test and failed to respond to department inquiries regarding the test.

Pierce County

In April 2008 the Nursing Commission charged registered nurse Sharleen A. Conner (RN00106814) with unprofessional conduct. She allegedly stole about 27 tablets of oxycodone from a person whom she was visiting and ingested some of the pills. She also allegedly failed to respond to department inquiries regarding the incident.

In April 2008 the Nursing Commission charged licensed practical nurse Alice N. Currie (LP00050270) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.

In April 2008 the Nursing Assistant Program denied the application of nursing assistant applicant Raksa Son (NA00198237). He was convicted of assault – domestic violence and two counts of protection order violation – domestic violence. Son failed to respond to department inquiries regarding the convictions. He cannot reapply for five years.

Snohomish County

In April 2008 the Nursing Assistant Program reinstated the credential of Ronald M. Nealon (NA00114454) and placed it on probation for 18 months. He must comply with the terms and conditions set against his credential.

Spokane County

In April 2008 the Nursing Assistant Program charged nursing assistant applicant Katherine A. Hardie (NA00200393) with unprofessional conduct. She was convicted of solicitation to commit possession of a controlled substance.

In April 2008 the Unlicensed Practice Program issued a notice of intent to issue cease and desist order to Susan Malmquist. She allegedly held herself out as a psychologist including signing documents and submitting billing. She does not hold a credential to practice as a psychologist.

Thurston County

In April 2008 the Nursing Assistant Program charged nursing assistant applicant Bobbi R. Calley (NA00196056) with unprofessional conduct. She was convicted of destruction of property, soliciting prostitution, criminal trespass, and obstructing a public servant. She allegedly failed to disclose the convictions on her application.

In April 2008 the Registered Counselor Program denied the application of registered counselor applicant Stephen E. Rhodes (RC00057042). He was convicted of assault and possession of methamphetamine. He cannot reapply for five years after his last conviction which took place in August 2004.

In April 2008 the Nursing Assistant Program denied the application of nursing assistant applicant James D. Vansyckle (NA00192801). He was convicted of assault. He cannot reapply until February 2010.

Whatcom County

In April 2008 the Registered Counselor Program charged registered counselor applicant Leslie J. Gesner (RC00059382) with unprofessional conduct. Her license to practice as a midwife in Washington was revoked with no right to reapply for five years.

Yakima County

In April 2008 the Nursing Assistant Program charged nursing assistant applicant Martina M. Ramirez Cerda (NA00200558) with unprofessional conduct. She was charged with theft and assault and was convicted of defying order to leave premises. She allegedly failed to respond to department inquiries regarding the charges and conviction.

In April 2008 the Nursing Commission charged registered nurse Heather L. Morse (RN00145303) with unprofessional conduct. She allegedly had sexual contact with a patient.

In March 2008 the Nursing Commission terminated the probation against the license of licensed practical nurse Suanne Wood (LP00047316).

Out of State

Idaho: In March 2008 the Office of Emergency Medical Services and Trauma System Program corrected the statement of charges against paramedic Julian Lindsay (ES00131072). He allegedly engaged in violent nonconsensual physical and sexual contact with a female which resulted in physical trauma.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
