  • Diabetes Education Program Application (110 KB - PDF)
    Hospitals and clinics with outpatient diabetes education programs are invited to apply for Medicaid reimbursement for diabetes education services provided to Medicaid clients.  Application requirements are explained in the application.
  • Hospitals with Medicaid Approval (27 KB - MS Word Doc)
    Hospital outpatient diabetes education programs which are approved for Medicaid reimbursement.
  • Clinics with Medicaid Approval (213 KB - MS Word)
    List of clinic outpatient diabetes education programs which are approved for Medicaid reimbursement.
