The Washington State Diabetes Plan, (PDF 167KB),  launched in March 2005, is a 20-page printed document containing 10 goals to be addressed over a five-year period. The plan outlines the relationship between the goals, specific activities, network organizations and the 10 Essential Public Health Services.

The Washington State Diabetes Network was founded to acknowledge the organizations and people working to address the state plan goals in the private, public, tribal, community and academic/training sectors, all of which contribute to diabetes prevention and control throughout the state. The network meets annually and communicates via email, as well as thru the Washington State Diabetes Connection.

The Washington State Diabetes Connection is a web portal that serves as a clearinghouse for the Washington State Diabetes Network, to foster collaboration and communication within the network. This portal is designed to house activities, ideas, best practices, links and resources relating to the Washington State Diabetes Plan, and to provide useful, credible information to people and organizations doing work to prevent and control diabetes among residents of Washington.

The Leadership Team of the Washington State Diabetes Network is a statewide coalition devoted to reducing diabetes and the resulting morbidity and mortality among Washington residents.  The Team operates as a voluntary collaboration of public, private, tribal, community and academic stakeholders in carrying out the network’s mission, as well as implementation of the Washington State Diabetes Plan.  For more information about the Leadership Team, please click here.
