The WISE Grant was a federally funded four year grant (6/1/01-8/31/05) to improve statewide systems for children with special health care needs and their families. Work of the grant focused on four goals: Care Coordination, Common Application, Integrated Data, and Blended Funding.

A variety of needs assessments were conducted to identify barriers to implementing each of the goals at both the local and state level.  Families were involved in all levels of the WISE Grant.  Recommendations for statewide system change based on the findings of the WISE Grant were submitted to state leadership in the summer of 2005.

Five pilot communities (Grant, Yakima, Island, Whatcom, and Cowlitz/Wahkiakum), chose one or more of the goals and implemented innovative strategies. The work of the pilot sites produced significant outcomes in all areas:

Agency/System Outcomes

  • Collaboration was increased among local agencies and organizations
  • Communication across agencies was increased
  • There was a significant increase in efficiency of local care system
  • There were changes in services or practices within local agencies

Parent/Child Outcomes

  • Children with special health care needs experienced an increase in access to services
  • Parents felt a decreased sense of isolation

Project CONNECT, a proposal for a common application model, was a direct outcome of the WISE Grant and includes an application portal which would provide web based access for families, service providers and others to learn about and apply for services, support and resources.

Children with special needs include children who have, or are at risk for, physical, developmental, behavioral, or emotional conditions and who also require health, social, and educational services beyond that required by children generally.
