Radioactive Materials Fee Codes

As stated on the Current Annual Fees page, below are the two digit fee code used by the Section and a short description of the licensed activities.

05	LARGE MANUFACTURER	For a license authorizing a single facility to use more than one curie of unsealed radioactive material in the manufacture and distribution of radioactive products or devices containing radioactive material. 246-254-070(1)(c)
06	SMALL MANUFACTURER	For a license authorizing a single facility to use less than or equal to one curie unsealed radioactive material or any quantity of previously sealed sources in the manufacture and distribution of products or devices containing radioactive material. 246-254-070(1)(d)
07	DECONTAMINATION	For a license authorizing decontamination services operating from a single facility. 246-254-070(1)(f)
08	HEALTH PHYSICS	For a license authorizing health physics services, leak test services, calibration services, and/or equipment servicing involving: Incidental use of calibration sources; Maintenance of equipment containing radioactive material; or Possession of sealed sources for purposes of sales demonstration only. 246-254-070(1)(i)

12	BROAD A (BIG)	For a license authorizing possession of atomic numbers three through eighty-three with maximum authorized possession of any single isotope greater than one curie. 246-254-070(2)(a)

28	LARGE LAB	For a license authorizing possession at a single facility of unsealed sources in amounts greater than: One millicurie of I-125 or I-131; or One hundred millicuries of H-3 or C-14; or Ten millicuries of any single isotope. 246-254-100(1)(a)

39	RHF-15 (IN-VITRO TESTING)	For persons registered to perform in-vitro testing pursuant to form RHF-15. 246-254-100(3)
40	RHF-20 (DEPLETED URANIUM)	For persons registered to possess depleted uranium pursuant to form RHF-20. 246-254-090(3)
60	RADIOGRAPHY - FIELD	For a license authorizing the use of radiographic exposure devices at temporary job sites but operating from a single storage facility. 246-254-090(1)(b)
61	WALK-IN IRRADIATOR	For a license authorizing possession of sealed sources for a walk-in type irradiator at a single facility. 246-254-090(1)(h)
64	REGISTERABLE GENERALLY LICENSED DEVICES	For a license authorizing the use, possession, and transfer of devices containing certain types of radioactive material as stated in WAC 246-233-020 (3) (k) at a single facility. 246-254-090 (4)
65	MICROLAB	For a license authorizing possession at a single facility of unsealed or sealed sources in amounts: Less than or equal to 0.01 millicurie of I-125 or I-131; or Less than or equal to one millicurie of H-3 or C-14; or Less than or equal to 0.1 millicurie of any other single isotope. 246-254-100(1)d)
66	NORM LAB	For a license authorizing possession at a single facility of large quantities of naturally occurring radioactive material in total concentration not exceeding 0.002 microcurie per gram. 246-254-100(1)(e)
67	STATIC ELIMINATOR	For a license authorizing possession of static elimination devices not covered by a general license. 246-254-090(1)(k)
68	OTHER, NOT SPECIFIED	For types of licenses or categories not covered by any other fee category. 246-254-070(3)

ADDITIONAL SERVICE FEES
RECIPROCITY	For reciprocal recognition of other agreement state, licensing state, or United State Nuclear Regulatory Commission licenses based upon the actual amount of radioactive material or type of devices being transported into Washington State or the type of service to be performed involving radioactive material. A fee equal to one hundred percent of the fee specified for the facility category. 246-254-110 (2)
FOLLOW-UP	For a second follow-up inspection, and each follow-up inspection thereafter, a fee of one hundred dollars per hour of direct staff time associated with the follow-up inspection, not to exceed one thousand two hundred fifty dollars per follow-up inspection. Hours are calculated in half-hour increments. 246-254-120 (1) (a)
ENVIRONMENTAL MONITORING VISIT	For each environmental cleanup monitoring visit, a fee of one hundred twenty-five dollars per hour of direct staff time associated with the environmental cleanup monitoring visit, not to exceed three thousand seven hundred fifty dollars per visit. Hours are calculated in half-hour increments. 246-254-120 (1) (b)
NEW APPLICATION	For each new license application, the fee of two hundred dollars in addition to the required annual fee. 246-254-120 (1) (c)
SS & D	For each sealed source and device evaluation, a fee of one hundred twenty-five dollars per hour of direct staff time associated with each sealed source and device evaluation not to exceed three thousand seven hundred fifty dollars per evaluation. 246-254-120 (1) (d)
AIR/ENVIRONMENTAL	For review of air emission and environmental programs and data collection and analysis of samples, and review of decommissioning activities by qualified staff in those work units, a fee of one hundred twenty five dollars per hour of direct staff time associated with the review. The fee does not apply to reviews conducted by the radioactive materials staff and does not apply unless the review time would result in a special service change exceeding ten percent of the licensee's annual fee. 246-254-120 (1) (e)
EXPEDITED LICENSE FEE	For expedited licensing review, a fee of one hundred twenty five dollars per hour of direct staff time associated with the review. This fee only applies by the mutual consent of licensee and affected staff, when a licensing request is taken out of the date order and processed by staff during non-work hours, and for which staff is paid overtime. 246-254-120 (1) (f)
PAYMENT OF FEES	The department shall process an application only upon receipt of the new application fee and the annual fee. 246-254-120 (3)
FAILURE TO PAY FEES	The department may take action to modify, suspend, or terminate the license or sealed source and device registration if the licensee fails to pay the fee for additional licensing and compliance actions billed by the department. 246-254-120 (4)
