Industrial Licensees

Industrial licensees are those individuals who perform activities involving sealed sources used in various types of devices.  Determine which of the following applications is appropriate for your use.  The application must be signed by a company official before you submit it for review.

• Industrial Radiography

The IR Application (PDF, 188KB) is DOH Publication 322-046 and the IR Instructions (PDF, 209KB) are in DOH Publication 322-047.

• Portable Gauge Users

The PG Application (PDF, 104KB) is DOH Publication 322-028 and the PG Instructions (PDF file - 118.40KB) are in DOH Publication 322-029.  Also available in the Microsoft Word version (MS Word, 88.5KB) with instructions (MS Word, 49.5KB).

• Fixed Gauge Users

The FG Application (PDF, 153KB) is DOH Publication 322-042 and the FG Instructions (PDF, 20KB) are in DOH Publication 322-045.

• Gas Chromatography Users

How to Determine your Application Fee for New Licensees

Find your fee category and the corresponding annual fee that most nearly matches your proposed operation from the Current Annual Fees.  It is highly recommended that you contact the appropriate Licensing Manager to avoid mistakes that could delay your licensing request. The total fee you must submit is the current annual license fee, plus the new application license fee of $200.
