 Step 1
Be:

Licensed, bonded, and insured as a Washington State General Contractor (L&I)

 Step 2

Have:

Minimum of two DOH certified Clandestine Drug Lab (CDL) decontamination employees, at least one of which must be a DOH certified CDL Supervisor

 Step 3 Submit:

DOH CDL Contractor application form (pdf format 54 kb) including a fee of $1125 for initial or renewal application.

    Contractor Certification lasts for 1 year.
