Migrant Farmworker Housing (MFH)

Initial Licensure:

First time applicants or applicants of new sites can receive the license application packet by contacting our office by mail, telephone, or email to request an application packet be sent or click on the application packet link below.

        Application Packet - Reg     Application Packet - Cherry Harvest     Fee

Return the application packet with:

  • The licensing fee is specified in WAC 246-358-990 or in WAC 246-361-990 for Cherry Harvest Camps.  See fees.  Include a check or money order made payable to "Department of Health."  At this time, we do not have a way to pay your fees on the web or over the phone.
  • Verification of approved water samples by Washington State Department of Health Drinking Water Program.
  • A copy of your Master Business License (also known as Uniform Business Identifier (UBI) number).  If you would like more information about Master Business License/Uniform Business Identifier (UBI) number, we suggest you contact the Washington State Department of Licensing (DOL) at their website (www.dol.wa.gov/business/) or call them at (360) 664-1400.

Each housing site must be inspected by our staff to determine that all health and safety standards are met.  After we receive your application packet, the water test results, and your UBI number, we will contact you for the site survey/inspection.

A license will be issued upon satisfactory completion of the following:

  • Initial application,
  • License fees,
  • Verification of approved water samples by Washington State Department of Health Drinking Water Program, and
  • On-site inspection conducted and the Department is satisfied the facility is in compliance with the law and rules, or if violations exist, corrections to them are satisfactorily addressed.
