Residential Treatment Facility

Complaints:

Contact our Complaint Investigation Unit to report any health and safety concerns you may have about a specific residential treatment facility or institutional facilities.

When contacting the Department of Health Investigation Unit, be prepared to provide specific information describing your concerns, such as the name and address of the  facility and the dates you stayed at the facility. 

Please understand the Department of Health’s Residential Treatment Facility Licensing Program has jurisdiction only over health and safety issues regarding a facility.  If your concerns are about misrepresentation, fraud or money, please call the Office of the Attorney General, Consumer Protection Division at 1-800-551-4636.  For criminal activities, please contact local law enforcement authorities.
