Prehospital Participation Grant Information

The following information and documents are provided to assist verified Washington State EMS agencies in applying for available prehospital Participation Grant monies.

Prehospital Participation Grant Documents 

1. Print and complete both the A-19 and the Survey Form.

   Participation Grant A-19

   Prehospital Participation Survey Form

2. Submit the completed A-19 with an original signature and the Survey Form to the address provided at the bottom of the Survey Form.
