The Licensing and Certification (L&C) Section of the Office of Emergency Medical Services and Trauma System (OEMSTS) licenses EMS agencies in accordance with RCW 18.73 and WAC 246-976-260 through 340.    The intent of this legislation is to provide minimum standards for ambulance and aid services, vehicles, and emergency medical equipment.

  The OEMSTS also provides verification for trauma care to EMS agencies in accordance with RCW 70.168 and WAC 246-976-390.  The eight EMS Regions each have an EMS and Trauma Care Council that provides a recommendation for trauma verification in accordance with their EMS and Trauma Care Plan and Patient Care Procedures. 

  The L&C Section of OEMSTS functions as the entity that establishes and maintains the integrity of the prehospital EMS system in Washington State.  We are expected to ensure the citizens have the most reliable EMS care. 

  The information that follows will provide you with the guidelines to determine whether an EMS license is required for your agency, and if so, the instructions and forms necessary to begin the licensure process.

Initial Licensure
Request for EMS license for the first time as a specific type: Aid (non-transport) or Ambulance (transport)
Relicensure/Reverification
Renewal of an existing license or verification at the same level of care and type of service.
Initial Trauma Verification
Request for verification, a higher form of licensure, for the first time at a specific level (BLS, ILS, or ALS) or type (Aid or Ambulance)

Air Ambulance Relicensure/Verification  
Renewal of an existing air ambulance service license or verification at the same level and type.

Initial Air Ambulance Licensure
Request for EMS license for the first time by an air ambulance service, whether interfacility or emergency. 

Vehicle Changes
Inform OEMSTS of any additions, removals or changes to vehicles licensed by your agency
Initial Air Ambulance Trauma Verification
Request for verification as an EMS air ambulance service providing emergency care.

EMS Agency Reports
Request custom reports for your licensed EMS agency.

 

Initial Licensure Process

 Step 1:   Review the cover letter for the Service/Vehicle Licensure Application.

Step 2:   Review RCW 18.71, 18.73 and WAC 246-976-260 through 340

Step 3:   Review the prehospital excerpts of the Regional Plan where you will be providing care.  For additional information on EMS regions, please read the Introduction and Regional Contracts information.

Step 4:   Complete the Service/Vehicle Licensure Application.

NOTE:  This form  MUST be printed out to complete and obtain the necessary signature.

 Step 5:   Submit the application with all the required information to the Licensing and Certification office (addresses on Page 4 of the application).

 

Initial Trauma Verification Process

Step 1:   Review the cover letter for the Service/Vehicle Licensure and Trauma Verification Application.

Step 2:   Review RCW 18.71, 18.73 and 70.168, and WAC 246-976-260 through 400

 

Step 3:   Review the prehospital excerpts of the Regional Plan for the region where you will be providing care.  For additional information on EMS regions, please read the Introduction and Regional Contracts information.

 

Step 4:  Review and follow the revised Verification Application and Evaluation Process.

 

Step 5:   Complete the Service/Vehicle Licensure and Trauma Verification Application.

 

NOTE:  This form  MUST be printed out to complete and obtain the necessary signatures (DOH will obtain the recommendation of the Regional Council).

Step 6:   Submit the application with all the required information to the Licensing and Certification office (addresses on Page 5 of the application).

 

Initial Air Ambulance Licensure Process

Step 1:   Review the cover letter for the Air Ambulance Service/Vehicle Licensure Application.

Step 2:   Review WAC 246-976-320.

 

Step 3:   Review the 2001-2003 Biennial Washington State EMS and Trauma Care Plan.  

 

Step 4:   Complete the Air Ambulance Service/Vehicle Licensure Application.

 

NOTE:  This form  MUST be printed out to complete and obtain the necessary signature.  

 Step 5:   Submit the application with all the required information to the Licensing and Certification office (addresses on Page 4 of the application).

 

Initial Air Ambulance Verification Process

Step 1:   Review the cover letter for the Air Ambulance Service/Vehicle Licensure/ Trauma Verification Application.

Step 2:   Review WAC 246-976-320 through 390.

 

Step 3:   Review and determine if you are in compliance with the current  Biennial Washington State EMS and Trauma Care Plan and be aware of the Patient Care Procedures for all regions within your response area.

 

Step 4:  Review and follow the revised procedure for evaluating applications for verification.

 

Step 5:   Complete the Air Ambulance Service/Vehicle Licensure/Trauma Verification Application.

 

NOTE:  This form  MUST be printed out to complete and obtain the necessary signature.  

Step 6:   Submit the application with all the required information to the Licensing and Certification office (addresses on Page 4 of the application).

 

Relicensure/Reverification Process

Step 1:   Review the Renewal Notice for the Service/Vehicle Relicensure Application.  This letter is sent to all licensed EMS agencies within 60 days of expiration. 

Step 2:   Request the Agency Information mentioned in the letter.  This includes your agency abstract, a list of the licensed vehicles, and a list of EMS personnel associated with your agency.

 

Step 3:   Complete the Service/Vehicle Relicensure Application.

 

NOTE:  This form  MUST be printed out to complete and obtain the necessary signature.

Step 4:   Submit the application with all the required information to the Licensing and Certification office (addresses on Page 3 of the application).

Air Ambulance Relicensure Process

Step 1:   Review the Renewal Notice for the Air Ambulance Service/Vehicle Relicensure Application.  This letter is sent to all licensed EMS agencies within 60 days of expiration. 

Step 2:   Request the Agency Information mentioned in the letter.  This includes your agency abstract, a list of the licensed vehicles, and a list of EMS personnel associated with your agency.

 

Step 3:   Complete the Air Ambulance Service/Vehicle Relicensure Application.

 

NOTE:  This form  MUST be printed out to complete and obtain the necessary signature. 

Step 4:   Submit the application with all the required information to the Licensing and Certification office (addresses on Page 4 of the application).

Vehicle Changes Process

Step 1:   Review the cover letter for the Vehicle Changes Application.

Step 2:   Request the Vehicle List mentioned in the letter.  This list shows the licensed vehicles we currently have on record for your agency.

 

Step 3:   Complete the Vehicle Changes Application.

 

NOTE:  This form  MUST be printed out to complete and obtain the necessary signature.

Step 4:   Submit the application with all the required information to the Licensing and Certification office (address on the bottom of the application).
