By submitting the required information listed above, you are attesting that the information above is accurate to the best of your knowledge. For most projects we will need a copy of your evacuation map (reduced floor plan) and approval from the local jurisdiction mailed to our office before we can complete the project close-out. (Certificate of Occupancy, signed off building permit etc.)

Once the pink card has been processed, the date of construction completion will become the official date of which the facility may bill for Medicare/Medicaid services.
