Complaints

Every health care agency or facility in Washington State is responsible to provide safe and competent care. Whenever possible, you should discuss any complaint or concern about you health care with the agency/facility administration. Usually the agency or facility has a process for handling and resolving issues and complaints. 

Anyone who has concerns about care received at an agency or facility regulated by the Department of Health Facilities and Services Licensing may contact our office by the toll free hotline number, mail or e-mail.

This number is on a 24 hour-7 day a week message machine. Registered nurses will return your call Monday-Friday 8 AM to 5 PM.
